Assessment of Different modalities of Circumareolar Augmentation Mastopexy in Middle aged Egyptian Females

Abstract

Background: Mastopexy on patients with small and medium-sized ptotic breasts presents one of the biggest challenges for plastic surgeons since mastopexy has opposite effects on the breast surface and parenchyma in these patients. These procedures aim to improve projection, decrease breast ptosis, and rejuvenate the patient.

Objective: to evaluate a group of female patients undergoing circumareolar breast augmentation mastopexy using implants and imaging methods in order to treat ptosis in volume-depleted breasts and produce a more youthful appearance, improved projection, and decreased ptosis of the breast.

Methods: In this prospective study, 30 middle-aged female patients with second- and third-degree ptosis or hypomastia underwent one stage breast augmentation with mastopexy technique via circumareolar approach at Kasr Al Ainy hospitals in Cairo and Beni-Suef university hospitals in Egypt. The patients were divided into two groups based on whether or not they want breast implants to boost breast volume. Records were kept of standard breast measurements, early and late issues, patient satisfaction, and outcomes.

Results: A total of 30 patients (60 breasts) underwent one stage mastopexy using circumareolar method over a period of 4 years. Preoperative testing revealed that 5 patients (16.66%) and 25 of 30 patients, respectively, had grade III ptosis and grade II ptosis. 6 of our patients had asymmetrical breasts, which caused a disparity in implant volume from side to side. Following surgery, 25 of the patients (or 84%) who underwent follow-up showed good outcomes, 3 (or 10%) exhibited good results, and 2 (or 6%) showed bad results, depending on age, implant design, NAC lift, and asymmetry.

Conclusion: Circumareolar augmentation mastopexy is quite helpful in producing a breast form that is acceptable in terms of aesthetics, especially in grade II ptosis where nipple elevation is 5 cm or less. Grade III ptosis, however, is more likely to develop NAC expansion if the NAC elevation is greater than 5 cm.
